Thank you for submitting the Application Form. This is the first step in the admission process. In order for your application to be reviewed, the following documents must also be submitted:

  1. Affidavit of Support. Please obtain an Affidavit of Support from your bank or other financial institution that shows your account balance. Then email that form to admissions@meridianschool.org or fax to 801-905-3179.
  2. Official Transcripts. Email a copy of the student’s transcript or last two years of school records to admissions@meridianschool.org or fax to 801-905-3179.
  3. Teacher Recommendation Form (click here). Please have the student’s English language teacher fill out this form. Send the completed form to admissions@meridianschool.org or fax to 801-905-3179.
