Volunteer Opportunity
Looking for a way to get those 40 volunteer hours? We need someone to clean the carpets underneath the lunch tables once a month.  We have a carpet cleaner available, you just provide the manpower (or womanpower!). Please contact Mrs. Rucker at the school if you are willing: krucker@meridianschool.org.

Engrade
Meridian School has established its school site at www.engrade.com/meridianadmin. On the home page, you can see all the Meridian teachers who are participating. Follow these instructions to access your student's grades:

1. Go to www.engrade.com/meridianadmin.
2. Click on Sign Up For Engrade.
3. Call the school receptionist at (801) 374-5480 to obtain your student's ID number.
4. Enter your Access Code as: engrade-meridianadmin-(student's ID number).
5. Set up your account username and password.
6. See grades for all your student's classes!

MPO News -

It's time for our fall Box Tops for Education drive! This has proven to be an effective (and relatively pain-free!) fundraiser for the MPO, which helps fund our Teacher's Support events and various student activities throughout the school year.
From 22 Sept - 10 Oct, students are encouraged to collect Box Tops for Education labels, place them in a recloseable plastic bag that has been labeled with the student's name and class, then deposit them in the collection box located near the reception desk. Each Friday, the labels will be counted and the winning class will receive a prize the following Monday. The class collecting the most Box Tops overall will earn a WOW day in October. Best of all, everyone benefits from the funds raised!

So - spread the work to relatives, neighbors and associates, far and near.On your mark, get set, COLLECT!

2008-2009 Information Sheets
Parents should download, print, and complete the forms that are applicable to their student. Forms are due to the school by the first day of classes. 
If you are having difficulty opening forms, you may need to upgrade your version of Adobe Acrobat.

Field Trip Permission Form: must be completed by every family

Health and Safety Form: must be completed by every family

Photo Consent Form: must be completed for each student

Student Computer/Internet Use Acknowledgement Form: must be completed for each student

Immunization Records: turn in copy of immunization record to school nurse or registrar

• Pre-Kindergarten: a student must meet the minimum immunization requirements of the ACIP for the following antigens: Diphtheria, Tetanus, Pertussis, Polio, Measles, Mumps, Rubella, and Haemophilus Influenza Type b prior to school entry.
• Kindergarten: meet the minimum immunization requirements of the ACIP prior to school entry for the following antigens: Diphtheria, Tetanus, Pertussis, Polio, Measles, Mumps, Rubella, Hepatitis B, Hepatitis A, and Varicella.
• 7th grade:  must also meet the minimum immunization requirements of the ACIP prior to seventh grade for the following antigens: Adult Tetanus/Diphtheria and Varicella.
